Camp Location:          Rising Star Soccer Fields (adjacent to Leb. Expo Ctr. on Evergreen Rd, Lebanon)

Camp Dates/Time:     Monday, July 21 – Friday, July 25, 2008 / Evenings 5:30 – 8:00 PM

                                    Registration on 7/21 from 4:30 – 5:15 pm

Camp Ages:                5 – 14 Girls and Boys (as of 8/1/08)

Camp Goals & Philosophy:

Leadership, teamwork, preparation and maximized effort are key principals.   Rising Star Soccer Camp will focus on developing your child’s fundamental soccer skills through progressive individual techniques and group tactics.  The week will culminate in a personalized evaluation on key skill areas to help identify your child’s progress and needs.  Throughout the entire week the kids will be learning and improving in a fun environment!

What To Bring To Camp:

• T-Shirts, shorts (shirts will be provided the first night)
• MUST wear shinguards underneath socks and molded soccer shoes
• Players should bring their own ball (not mandatory) and a drink

Payment Information:

Affordable $60 per player cost for week’s training and camp shirt. ($50 per player for 3 or more)

Return completed form no later than JUNE 30, 2008 with a check made payable to “Rising Star Soccer Camp 2008”

Recognizing the possibility of physical injury associated with soccer and in consideration for the EPYSA accepting the registrant for its soccer programs and activities (the "Programs"), I hereby release, discharge and/or otherwise indemnify the EPYSA, its affiliated organizations and sponsors, their employees and associated personnel, including the owners of fields and facilities utilized for the Programs, against any claim by or on behalf of the registrant as a result of the registrant's participation in the Programs, and/or being transported to or from the same, which transportation I hereby authorize.
